What do I need to bring to my first appointment?

You will need your insurance card, a referral from your doctor (if we do not already have a prior authorization), and comfortable clothing. It would also be helpful to bring whatever shoes you wear for exercising or walking. If coming from work, feel free to bring a change of clothes and use one of our private treatment rooms.

How long are initial evaluations and follow up appointments?

For your first visit, expect to be at the clinic between 40-75 minutes. You will also need to arrive at least 30 minutes early in order to fill out the necessary paperwork, so that the therapist can spend the full time scheduled with you. Each subsequent visit will last between 30 - 60 minutes depending on the therapist's treatment plan

Is parking available on-site?

Parking is available in the lot behind the building. Upon entrance inform the security guard you are attending a therapy appointment and you will be directed to a designated First PT parking space.

Will I owe any money at the time of my visit?

Yes, you will be responsible for paying any co-pays due at the time of your visit. We will bill your health plan for all services and will bill your directly for any deductibles or uncovered services.

How do I cancel or make a change to a scheduled appointment?

If canceling or changes are needed, our policy at First PT is a minimum of 24 hours notice. If changes need to be after hours, please leave a message for the staff to receive in the morning.
